Hearth Connection is a nonprofit that acts as an intermediary between government and local nonprofits to end homelessness in Minnesota.

Hearth Connection offers Supportive Housing through a network of providers in three regional long-term homeless projects. Our program yields tremendous success for people experiencing long-term homelessness.

Our evaluation shows that government spending dramatically shifts from expensive, 'bad' costs (emergency rooms, jails, detox etc) to productive, 'good' investments (primary health & mental health care, medicine etc) when people have stable housing and support. Hearth Connection has supported 2,300 men, women, youth and children.


We have demonstrated it is possible to end homelessness one participant at a time.

  • April 28, 2010:  On behalf of the Hearth Connection Board of Directors, we are delighted to announce that Richard Hooks Wayman will become Hearth Connections Executive Director on June 15, 2010.  Rich has been the senior youth policy analyst at the National Alliance to End Homelessness since 2006. To read more about Rich, please view the official announcement from Hearth Connection's Board of Directors. We believe his passion for our mission and his commitment to collaborative work will bring new energy and momentum to ending homelessness in Minnesota. Welcome Rich!

  • February 1, 2010:  Hearth Connection is pleased to announce that 14 people now live in Delancey Apartments. Opening in May 2009, Delancey Apartments provides permanent supportive housing for people with histories of chronic homelessness and difficulty maintaining housing. The project is the result of many collaborators, including Hearth Connection, Guild Incorporated, and Project for Pride in Living (PPL). Hearth Connection and Guild Incorporated have partnered for over a decade, working together to help individuals break the cycle of homelessness. Delancey Apartments is the most recent product of their partnership, which began with the development of the Supportive Housing and Managed Care Pilot Project.

  • January 22, 2010: Jennifer Ho, Executive Director of Hearth Connection, has accepted a position in Washington DC as one of three Deputy Directors at the U.S. Interagency Council on Homelessness. Her last day at Hearth Connection will be Friday, January 29. Congratulations Jennifer!  We will miss you.

  • December 28, 2009: The recording of Hearth Connection's Minnesota Supportive Housing and Managed Care Pilot Webinar is now available. The Webinar, held on December 15, presents the findings from the study that demonstrated how participants' lives improved and supportive housing caused desirable shifts in costs associated with their care.
