Data-driven Outcomes

Hearth Connection is a national leader in documenting results and using data to drive better outcomes for participants and to increase the public’s and policymakers’ understanding of homelessness and what can be done to end it. Hearth Connection has developed systems to be able to track participant outcomes and account for taxpayer investments. These tools allow Hearth Connection to advocate for additional resources based on facts, results and documented evidence. This information also allows individual supportive service providers to analyze their own performance compared to other organizations. We can focus on making improvements that will create the greatest efficiencies in systems and the biggest impact on participant outcomes. Ultimately, a data-driven system for ending homelessness will give participants informed choices about different supportive housing providers. When the evaluation of the Pilot is complete, Hearth Connection will disseminate these results and learnings both locally and nationally to increase understanding and investments in supportive housing.

Hearth Connection’s commitment to data and results gives all stakeholders a central point of accountability for monitoring the effectiveness of these projects focused on ending long-term homelessness. Hearth Connection could not accomplish these objectives without the active assistance of supportive service partners who collect data and help interpret results.
