County and tribal band leaders outline the scope of the project and provide oversight. Their guidance helps shape the regional projects, resolve system-level barriers, and maintain accountability.

Hearth Connection coordinates funding, reporting, advocacy, and day-to-day management for the regional projects. By building new partnerships and leveraging funds, the organization gathers and delivers the resources necessary to provide housing and services for participants. Hearth Connection reports outcomes to funders, partners, and the state.

Service providers from local nonprofits develop flexible service plans that evolve with participants' needs. They work with households to:

  • find and maintain housing
  • set goals
  • access health care
  • expand educational, employment, and life skills

Offering individualized support in either site-based or private market apartments, service providers meet participants where they're at. Literally.
