Who We Are

Hearth Connection acts as an intermediary, working with government and community social service agencies to help people with long histories of homelessness.

Our Mission: To build relationships, resources, infrastructure and understanding, so that no one in Minnesota remains homeless, and people who were homeless can prosper. 

  • Hearth Connection secures, distributes and leverages government funds for supportive housing. Over the years, we have developed effective relationships and broadened our support.
  • Hearth Connection innovatively combines different streams of funding for flexible and effective use by supportive housing providers. Our in-house software, Co-Pilot, effectively corroborates results for our funders. We take a lead role in ensuring compliance with each investor and funding source.

  • Hearth Connection bridges housing, social services and health care with supportive housing. We contract with community agencies to carry out programs based on best practices
  • We support our partner providers with training and guidance on best practices, reporting and compliance.
