Who We Are

Hearth Connection manages a network of supportive housing providers that includes mobile supportive service providers that use predominantly private market scattered-site housing and site-based supportive housing communities. Over 1,500 people have participated in our programs since we started in 2001. There are over 1,000 people in our programs today, including over 300 single adults, 200 families with children and 35 unaccompanied youth. Our supportive service partners have an excellent track record helping participants find housing, keep housing, and use stable housing as a foundation for recovery.

  • Hearth Connection increases the amount of resources to break the cycle of long-term homelessness, leverages funding from local, state and the federal government, for both housing and services, and provides the tools and systems needed to manage funding from multiple sources.
  • Hearth Connection is a national leader in documenting results and using data to drive better outcomes for participants and to increase the public’s and policymakers’ understanding of homelessness and what can be done to end it.
